Statement of Understanding/Student Learning Plan Form (SOU/SLP)
In accordance with Washington State Alternative Learning laws, ALL students taking HS Online Options courses are required to complete and submit a Statement of Understanding/Student Learning Plan form (SOU/SLP).

Instructions to Sign & Submit the Form:
1. Download, print & fill out the SOU/SLP above.
Submit the printed form by completing signatures & returning it to the student’s School Counselor, either in person or by scanning and sending via email.

2. Use the fillable pdf feature by opening the document, clicking on any field, choose create a copy, fill in information in each field, click in the sign field, choose e-sign, sign & re-save.
Attach fillable & digitally signed form to an email & send.
